By opting in to receive SMS messages from Hampton Home Care Inc, doing business as Hampton Homecare (“Hampton Homecare,” “we,” “us,” or “our”), you agree to the following terms and conditions.
1. SMS Messaging Service
By providing your phone number and checking the consent box on our patient intake form, or by giving verbal consent during a phone call with our staff, you consent to receive SMS text messages from Hampton Homecare concerning:
Appointment scheduling and reminders;
Order and delivery updates;
Equipment resupply notifications;
Troubleshooting and support follow-ups; and
Patient satisfaction surveys.
Message frequency varies. Message and data rates may apply. Reply HELP for support. Reply STOP to opt out.
2. How You Opt In
Hampton Homecare obtains SMS consent in one of two ways:
Online patient intake form: Through the patient intake form available at https://www.hamptonhomecare.com/forms/patient-intake , where you provide your mobile phone number, check an unchecked-by-default consent checkbox displaying the SMS disclosure, and sign the form by typing your full name.
Verbal consent: During your intake or equipment setup phone call, a Hampton Homecare representative reads the SMS disclosure aloud and records your agreement in our patient management system.
After your consent is recorded, you will receive a one-time confirmation SMS.
3. Message Frequency
Message frequency varies based on your care needs. Typical frequency is two to four messages per month, with up to one to five messages per week during active intake, appointment scheduling, delivery, or resupply periods.
4. Message and Data Rates
Message and data rates may apply based on the terms of your mobile carrier and wireless service plan.
5. Privacy Policy
Your information will be handled in accordance with our Privacy Policy .
Mobile information and SMS consent will not be shared with third parties or affiliates for marketing or promotional purposes.
6. Opt-Out Instructions
You may opt out at any time by replying STOP to any SMS message. For support, reply HELP, call us at (866) 229-5418, or email [email protected] .
7. Eligibility
By opting in, you confirm that you own or are authorized to use the phone number provided and that you are at least 18 years old.
8. Liability
Hampton Homecare is not responsible for charges, errors, failed delivery, or delays in SMS delivery caused by your mobile carrier or third-party service providers.
9. Consent Is Not a Condition of Purchase
You are not required to opt in to SMS messaging to receive equipment, supplies, or services from Hampton Homecare. Your consent to receive SMS messages is not a condition of any purchase or service.
10. Data Sharing
Text messaging originator opt-in data and consent will not be shared with third parties for marketing or promotional purposes.
We may share SMS opt-in or consent status with service providers, including messaging platform providers, phone companies, and other vendors, solely as necessary to operate and deliver the messaging service.
